Instructions For Completing
I-9 Form
You must have the correct original documents to show to the staff person completing the form. A driver's license, state picture ID or passport, and your Social Security Card (MANDATORY with signature) must be valid.
This document can be filled out online; however, you must print the document after it is completed for the signature portion. This will be brought to the Student Placement Office, located in the Student Union, along with the other required documents BEFORE you can begin working.

All student employees, upon being hired, must complete Section 1 of the I-9 Form. They must print or type their complete name, address, date of birth, and Social Security Number. The block must be checked that correctly indicates the student's immigration status. Most students will check block one. However, if the second block is checked, the student's Alien Registration Number must be provided. If the third block is check, the student's Alien Registration Number or Admission Number must be provided, as well as the date of expiration of that status, if it expires. All students must sign and date the form.

Employers (PERMANENT MUSKEGON COMMUNITY COLLEGE) must complete Section 2 by examining evidence of identity and employment eligibility:

- Check the appropriate box for List A or List B and C (most student will have easiest accessibility to driver's license and Social Security Card);
- Record the type of document used and any applicable identifying numbers.
- Sign the certification section. This signature certifies that the signee has examined the required documents. A student or temporary employee cannot certify the I-9 Form.
- Print only page 3 of the I-9 Form

The original completed I-9 Form must be forwarded to the Student Employment Office.

A paycheck will NOT be released without the I-9 Form on file in the Student Employment Office. |
Note: Employing units should not keep copies of the completed I-9 Form. The original must be on file in the Student Employment Office.
